Human error by flight crew primary cause of Shah Alam plane crash, says AAIB report

The Air Accident Investigation Bureau (AAIB) final report on its investigations on an air crash involving a private jet at Elmina township in Shah Alam in August last year has concluded that human error by flight crew was the primary cause of the accident.
